Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likley to cause death or serious physical harm to employees in that employees were exposed to: (a) Jobsite - Crushing hazards. An elevator car was not provided with car gates or doors at each entrance [American National Standards Institute/American Society of Mechanical Engineers A17.3-1990 section 3.4.2]. On or before July 13, 1993, at the freight elevator, employees who worked or rode on the elevator were exposed to being caught or crushed between the landing sills and the moving elevator car, which was not provided with car gates or doors at each entrance. ABATEMENT NOTE: Among others, a feasible method to eliminate or to materially reduce this hazard is to comply with ANSI/ASME A17.3-1990, section 3.4.2 and provide car gates or doors at each entrance to the car of the freight elevator.
