Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employeesin that employees were
exposed to:
(a) The hazard of being crushed by the moving elevator car of the
OTIS freight elevator and to falls down the elevator hoistway at
Sun Mountain Sports in the east building.  Among other methods,
feasible and acceptable abatement methods to correct the hazards
are:
1. All hoistway entrances must be provided with hoistway doors or
gates which guard the full width of the hoistway to a height of at
least 6 feet.  The hoistway doors or gates must be provided with
mechanical locks and electrical controls or with interlocks which
will prevent movement of the car unless all gates or doors are in
the closed position and will prevent the gates or doors from being
opened from the hoistway side unless the car is at that landing.
2. The capacity plate must correctly reflect the elevator capacity
as limited by the winding drum motor.
3. The distance from the landing sill to the hoistway doors/gate
must be limited to 4 inches.
