General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:   The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause  death
or serious physical harm to employees in that employees were exposed  to
numerous hazardous conditions such as, but not limited to, lack of  safe
access and proper fall protection at such  locations  as  the  chop  saw
grizzly and bin sorter, unguarded  machinery,  catwalks  and  stairways,
unguarded electrical equipment and employees were not instructed in  the
recognition  and avoidance of unsafe conditions, practices and the regu-
lations applicable to their work environment:
(a)  Establishment - On March 27, 1991 and at times  prior
thereto, new employees were not indoctrinated  on the
hazards of their jobs prior to assignment.
Among other methods, one feasible and  acceptable  abatement  method  to
correct this hazard is:
(1)  Develop detailed job hazard analysis for all  produc-
tion jobs.
(2)  Develop job specific  indoctrination  guidelines  for
supervisors to use in training new employees.
(3)  Periodically   review   and  update  all  job  hazard
analysis.
(4)  Retrain employees if changes are made during periodic
review of job hazard analysis.
