Section 5(a)(1) of the Occupational Safety and Health Act of 1970:   The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause  death
or serious physical harm to employees in that employees were exposed  to
crushing injuries:
(a)  Smith Creek Timber Sale - On or about June 6, 1991 and
at times prior thereto, ample  space  to  safety  park
skidder was not provided on Spur Road  to  chute  area
landing.  Employee was engaged in setting  chokers  on
below Spur Road, while  skidder  was  parked  on  same
grade  resulting  in  skidder  sliding down grade  and
crushing employee.
Among other methods, one feasible and  acceptable  abatement  method  to
correct this hazard is:
Prepare and arrange landing  area  to  safely  operate
equipment in  accordance  with  Idaho  minimum  safety
standards  and  practices  for  logging,   Chapter  M,
Standard 27, Section (1)(g).
5) WHEN LEAVING AN OPERATORY FOLLOWING A DENTAL PROCEDURE, GLOVES SHOULD
BE REMOVED AND DISCARDED BY HAND WASHING. IF CONTAMINATED GLOVES ARE
LEFT ON THE HANDS, ANOTHER PAIR OF GLOVES SHOULD BE WORN OVER THEM SO
THAT CONTAMINANTS ARE NOT LEFT ON OTHER SURFACES CONTACTED.
6) MECHANICAL DEVICES SHOULD BE PROVIDED FOR RECAPPING NEEDLES AND
ELIMINATE THE "SCOOP" METHOD OF NEEDLE RECAPPING.
7) HANDPIECES SHOULD BE FLUSHED AT THE BEGINNING OF EVERY DAY AND AFTER
EVERY PATIENT.
8) THE USE AND MAINTENANCE OF WATERLINES AND CHECK VALVES SHOULD BE
CHECKED AND DOCUMENTED FOLLOWING THE MANUFACTURERS' GUIDELINES ON
THE SYSTEM.
