General-duty citation text
Section 5(a)(1) of the Occupational Safety adn Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential injuries from: (1) elevator collapse, and/or (2) falls from unguarded floor areas and elevator platforms: a) Foundry freight elevator was not installed in accordance with American National Standard Safety Code for Elevators (ANSI A17. 1- 1960 NOTE: See appropriate rule noted below): 1) A single wire rope was used to hoist car. The minimum number of wire ropes shall not be less than two (Note: Minimum diameter shall be one-half inch). See Rule 212.4. 2) Safety devices to prevent free fall were not installed. safeties shall consist of a device which applies pressure to steel guide rails and activated based upon a set governor speed. See Rule 205.1 and 205.5. 3) Elevator shaft was not equipped with steel guide rails. See Rule 203.2. 4) Elevator car was not enclosed. Car was open on top and entrance/ exit sides. See Rule 204.1a. 5) Elevator car was not equipped with car doors and landing gates. These door/gates shall be equipped with electrical contacts. See Rule 204.4. 6) Passengers were permitted to ride freight elevators (Rule 207.4), which did not have the equivalent rating as required for passenger elevators (Rule 207.1), and the car was not equipped with doors and gates meeting the requrements fo Rule 204.6c and 204.6d. 7) Elevator car was not equipped with terminal stopping devices (limit switches) meeting the requirements of Section 209. NOTE: This is not a conclusive list of deficiencies for this elevator. Among other methods, one feasible and acceptable abatement method to correct this hazard is: 1) Replace elevator in accordance with current ANSI A17.1 series, or 2) Remove elevator from service and use a high rise forklift to raise and lower material from second floor storage area. NOTE: Option 2 above would require guardrails around open platform area.
