Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of becoming intangled in exposed, rotating parts which could result in fractures, crushing, and/or death: a) On or about 6/7/91 on the turbine check floor of the Goodyear Co-Generation Plant - Employees using Ridgid 535 pipe treading machine to thread pipe and the only device for shutting off the power was an on/off switch.
