free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
A) Facility: An employee was standing on a wooden pallet (without
guardrails and without it being secured to the forklift) and was
being lifted by a Baker forklift to a height of approximately 8 1/2
feet above a concrete floor.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to have the employee work from either a scaffold
or a ladder.
