Section 5(a)(1) Employees were expoed to fall hazards which could result in serious physical harm to death during sandblasting and spraypainting operations while working from a scaffold which was suspended from an overhead bridge metal via hangers, an assembly not approved by a duly constituted and recognized authority for their use as required by the Anerican National Standards for Safety Requirements (ANSI-A10.8,1988): a) Jobsite: On 5/11/90 and 5/14/90 an employee was exposed to fall hazards and serious physical harm or death while working from a suspended scaffold supported by metal hangers which is not a recognized scaffold support system design.
