General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees on the second floor were exposed to fall hazards when the carrier was on the first floor and crushing injuries if the carrier were entering the landing zone on the first floor. A feasible and useful method to correct this hazard, among other methods is to provide mechanical interlocks on the landing gates to prevent them from being opened unless the carrier has stopped at that level. (a) FG Department: The landing gates on the inclined reciprocating conveyor were not mechanicall interlocked.
