Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to serious injuries while working in proximity to an unguarded engine fan. A feasible method to correct this problem would be to install the proper engine fan guard. A) Jobsite - Pier area North side: Employee was exposed to serious injury while working in close proximity to an unguarded engine fan on the Warner Sweezy gradall G800.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an overhead hazard while engaged in hoisting materials by a crane in which the cable clips were improperly installed. Among other methods one feasible and acceptable method would be be to remove the dead end of the hoist cable from the line end. A) Jobsite: South barge: Employees exposed to serious overhead hazard while hoisting material by a crane in which the cable clips for the hoist cable were not installed properly on the line belt truck crane.
