Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: FALL HAZARDS WHEN ENTERING SITES CONTAINING HATCH COVERS AND WHERE THE ACCESS LADDER DID NOT EXTEND ABOVE THE HATCH OPENING. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS THE EXTENSION OF THE SIDE RAILS OF THE LADDER ABOVE THE GRADE LEVEL FACILITATING ENTRANCE AND EXIT OF THE SITE. (a) MAIN & SUFFIELD STS: (b) REED STREET: (c) VALLEY: THE ENTRANCE TO THE SITE CONTAINED A HATCH OPENING WHICH DID NOT PROVIDE FOR MEANS OF A GRAB BAR OR RAILS AS A MEASURE OF FACILITATING ENTRANCE AND EXIT OF A LADDER WHICH DOES NOT EXTEND ABOVE GRADE LEVEL.
