Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) Grant Street, Grant Building, Pittsburgh, PA; An employee engaged in
window washing activities from a boatswain's chair was exposed to a
fall hazard of up to 10 floors in that he was using a Sky Genie
descent control device, a piece of equipment operational in the down
direction only. The device is prohibited by ANSI/ASME A39.1 - 1987,
section 11.3
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to have the employees work from a two point
suspension scaffold or work off a unit meeting all applicable provisions
of ANSI/ASME A39.1 - 1987.

a) Jobsite; An employee engaged in window washing activities was exposed
to a fall hazard of up to 10 floors at the Grant Building, 330 Grant
Street, Pittsburgh PA. The seat harness did not bear an
identification mark either on a permanent tag or indelibly printed
into the unit identifying date of manufacture and that testing
requirements as specified under ANSI A10.14 have been met.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is the use of a safety belt/harness meeting all
applicable requirements of ANSI A 10.14.

that he was not trained in the use of the Sky Genie descent only
control device. ANSI/ASME A 39.1 - 1987, section 6.1.4 requires
individuals to be trained in the use of boatswain's chairs.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to have the employees work from a two point
suspension scaffold and be trained in the proper and safe use of the
equipment.

that his safety line was not protected from abrasion/damage as it
was hung at a right angle over the parapet wall.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to place a rubber hose or similar device around
the line to protect them from abrasion/damage.
