FREE FROM RECOGNIZED HAZARDS THAT WERE CAUSING OR LIKELY TO CAUSE DEATH
OR SERIOUS PHYSICAL HARM TO EMPLOYEES IN THAT EMPLOYEES WERE EXPOSED TO:
A) 2/8/91, EMPLOYEES WERE EXPOSED TO FALLS IN THAT EMPLOYEES WERE
PERMITTED TO LOAD, UNLOAD AND WORK NEAR FREIGHT ELEVATOR IN MAIN
BUILDING, WITHOUT ADEQUATE PRECAUTIONS TAKEN TO PREVENT THE OPENING
OF THE HOISTWAY DOOR FROM THE LANDING SIDE UNLESS THE CAR IS WITHIN
THE LANDING ZONE AND IS EITHER STOPPED OR BEING STOPPED IN THAT:
1) ELEVATOR ACCESS DOORS (OR "HOISTWAY DOORS") ON ALL FLOOR AT TIME OF
ELEVATOR MOVEMENT, WERE NOT EQUIPPED WITH PROPERLY FUNCTION HOISTING
UNIT SYSTEM DOOR INTERLOCKS. ELEVATOR ACCESS DOOR ON THE FOURTH,
THIRD, SECOND FLOOR, MAIN DOOR ON THE FIRST FLOOR AND BASEMENT DOOR
WERE EQUIPPED WITH MAGNETIC SWITCHES.
