Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Employees were permitted to load, unload & work near RKI Drum Elevator without adequate precatuions being taken to prevent the elevator platform from moving, when it should not, in that: The elevator's access doro had been completely removed, thus leaving the elevator without a door & at the time of elevator movement, the access door was not equipped with a functioning electric contact interlock switch. These conditions existed on 9/12/90. Feasible & acceptable abatement methods, among other, to correct these hazards are: (1) Repair or replace electric contact switches, (2) Install platform access door, (3) Absolute prohibition of any by-passing, jumping, removal (or like action) of any electric switches or interlocks or removal of the access door or removal of any safety device, and (4) Prohibition of use of any elevator needing repair or a malfunction.
