Section 5(a)(1) of the Occupational safety and Health Act of 1970:  The
employer did not furnish employment and place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that,  on October 4, 1991, a
fire began at the roof level of the JFK Federal Building, Boston, MA
and the following conditions existed:
(a) JFK roof level:  The wooden cooling tower located on the roof
was not provided with a sprinkler system as required by NFPA 214
Section 5-1.1, 1988.
One method among other methods to abate the condition is to install a
sprinkler system in the cooling tower area.
(b)  JFK roof level:  The wooden cooling tower located on the roof
was not provided with a heat detection system as required by NFPA
214 Section 5-2.8.2 1988.
One method among methods to abate the conditions is to install a heat
detection system in the cooling tower.
(c)  JFK roof level:  The roof of the building did not have adequate
water pressure and volume of water to properly fight the fire which
took place in the cooling tower area as required by NFPA 14 Section
5-3.1 1984.
One method among other methods to abate the conditions is to install a
fire pump within the building to provide and maintain the water pressure
and volume as neccessary throughout the building.

or serious physical harm to employees in that, the fire door assemblies
were not adequately rated for their use:
(a)  JFK:  The labels on the fire rated doors in stairwells 1
through 7 were painted over preventing the authority having
jurisdiction from determining if the fire rating of the doors were
adequate for their use, as required by NFPA 80 Section 1-5.1 1984.
One method among other methods to abate the condition is to locate the
neccessary documentation which would provide the actual rating of the
fire rated doors in stairwells 1 through 7.
(b)  JFK:  The door frames in stairwells 1 through 7 were not
is the frames had a fire rating adequate for their use as required
by NFPA 80 Section 2-5.1 1984.
One method among other methods to abate the condition is to locate the
neccessary documentation which would indicate that the door frames were
in fact fire rated frames.
(c)  JFK:  The lock and latch sets used on fire rated doors in
stairwells 1 through 7 were not fire rated panic hardward.
One method among other methods to abate the condition is to replace the
existing locks and latch sets with fire rated panic hardward.
(d)  JFK:  The fire rated doors leading to starwells 1 through 7
were not equipped with fire rated panic hardware as required by NFPA
101 Section 8-2.2.2.3 1991.
One method among other methods to abate the condition is to install
panic hardware on the fire rated doors.

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that, employees were exposed
to smoke and fire passing through the unsecured access door(s).
(a)  JFK:  The 4 access doors located between the 1st and 3rd floors
of stairwells 1 and 2, were not equipped with labeled latching
devices as required by NFPA 80 Section 10-1.1 1984.
One method among other methods to abate the conditions is to install
fire rated latching devices.
(b)  JFK:  The 4 access doors located between the 1st and 3rd floors
of stairwells 1 and 2, were not self-latching as required by NFPA
80 Section 10-1.2 1984.
One method among other methods to abate the condition is to install
self-latching devices.
(c)  JFK:  The 4 access doors located between the 1st and 3rd floors
of stairwells 1 and 2, were not self closing as required NFPA 80
Section 10-2-2 1984.
One method among other methods to abate the conditions is to install
door closures on all the access doors.

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees, in that, employees were exposed
to smoke and fire propagating through hallway(s):
(a)  JFK:  The opposite ends of the horizontal exit located on the
3rd floor between the high and low rise buildings were not equipped
with 1 1/2 hour fire rated automatic or self closing doors to
prevent the spread of smoke and fire from one building to the other
as required by NFPA 101 Section 5-1.3.1 (b) 1988.
One method among other methods to abate the condition is to install
1 1/2 hour fire rated automatic or self closing doors at each end of
the horizontal exit.

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees, in that, the signs within the
stairwell enclosures were inadequate:
(a)  JFK:  The existing signs located in stairwells 1 through 7 were
inadequate in that the signs did not indicate the terminus of the
top and bottom of the stairwells and the floor level of exit
discharge.
Also the existing signs were not located 5 feet above the
landing and readily visable when the doors were in the open and
closed positions as required by NFPA 101 Section 5-2.2.6.6 1991.
One method among other methods to abate the condition is to affix
appropriate signs at each floor landing of stairwells 1 through 7.
