Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to working conditions and recognized hazards of being overcome by toxic gas and/or oxygen deficient atmospheres while entering into and working inside a pit 82 feet deep in the sewage pump house. (a) Building No. 50, sewage pump house, the employer did not ensure that employees followed a confined space entry procedure when required to enter an 82 foot deep pit to perform valve repacking and monthly greasing. Among other methods, one feasible and acceptable abatement method to correct this hazard is to establish and enforce adequate confined space entry procedures, such as those stipulated by ANSI Z117.1-1989. These procedures include developing a program that includes a permit system, atmospheric testing, an assigned attendant who keeps constant direct communication during the time the employee is inside the confined space, a program of isolation and lockout, emergency response procedures including rescue equipment, such as harnesses and lifelines, and training of employees required to enter the confined space and training of standby attendants.
