Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of explosion/detonation of DIGL-RP propellant and/or 1910.119(j)(2) The employer did not establish and implement written procedures to maintain the on-going mechanical integrity of process equipment.: a) A Line mixer #10; Employees walking and working near mixer #10, were exposed to the hazard of fire/explosion causing or likely to cause death or serious physical harm. Mixer operators were utilizing mixer # 10 to mix DIGL-RP which had been stored in the mixer for approximately 60 days. The mixer had not received routine maintenance/cleaning due that period. The build up of hardened dry propellant inside the glands of the mixer created a hazardous condition during the mixing operation. One reasonable and acceptable abatement method to correct this hazard would be to follow the monthly cleaning and inspection procedure schedule as described in the CARE AND MAINTENANCE section of the Hercules Inc., Kenvil Works General Operating Procedures 3SMK-11, March 3, 1994, which specifies the cleaning and oiling of the mixer glands every thirty (30) days. Violation occurred on or about 06/28/94
