Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Employee(s) operating a freight elevator and/or riding a freight elevator
and/or working in the
vicinity of a freight elevator were exposed to the hazard of falling into
the elevator hoistway
and/or being crushed by the elevator cab due to the hoistway door on the
first floor "North" side
of the hoistway not having a functioning interlock device, and on the
first floor "South" side of
the hoistway not having an interlock device or being permanently locked
out
from the inside of
the hoistway.
Among other methods one feasible and acceptable abatement method to
correct this hazard is
to:
1.  Equip the freight elevator with a hoistway unit system hoistway door
interlock.

Employees were exposed to the hazard of being caught and/or crushed
between the elevator and
shaftway walls or sills.  Car door or gate was not provided on the
elevator car.
Among other methods one feasible and acceptable method to correct this
hazard is to:
1) Provide the freight elevator car with a car door or gate with a car
door or gate electric
contact.
