Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment that were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employee was exposed to THE HAZARD OF FALLING: a)North side of building, on or about 5/10/00: a manual boatswain's chair was used in a window cleaning operation at a maximum height of 98 feet. ABATEMENT NOTES: Ensure that manual boatswain's chairs are not used where the height of suspension exceeds 75 feet per American National Standards Institute, Safety Requirements For Window Cleaning, ASME/ANSI A39.1-1987, paragraph 6.1.2. Abatement certification must be submitted for this item. b)North side of building, on or about 5/10/00: Emergency decent equipment(equipment which is operational in the down direction only) was used for a window cleaning operation. ABATEMENT NOTES: Ensure that emergency decent equipment is not used for window cleaning operations per American National Standards Institute, Requirements For Window Cleaning, ASME/ANSI A39.1-1987, paragraph 11.3. Abatement certification must be submitted for this item.
