General-duty citation text
Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazard: a) Employees working in the plate steel sheet fabrication area of the old warehouse were required to use a 3 Ton, remotely controlled, electric, wire rope, underhung, trolley hoist, to lift/position heavy metal, weighing as much as 4000 lbs, and were being exposed to the hazards related to impacts of these loads, in that, the hoist lifting hook was found to have a throat opening of 3 7\16", a condition that exceeded the manufacturers recommended maximum of 2 1\4" by 380 These conditions were further exacerbated due to combined use of a double plate hook alloy chain sling assembly, found with coupling links (Cold Shuts) that were defective and unapproved as used. b) Frequent and Periodic inspections of underhung trolley hoists were not being scheduled, and written records of repairs and maintenance were also not being maintained in sufficient detail to assure hoisting equipment is safe for employees' use. A feasible and acceptable method to correct these conditions would be to; 1) replace the defective hook, and 2) implement scheduled inspections of all hoisting equipment in accordance with the manufacture's recommendations and document such repairs and maintenance.ons
