Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: State E, Well No. 20: Employees working on a Franks Explorer III, #69 well servicing rig were exposed to injury due to failure of hydraulic equipment on the rig such as hydraulic support legs and derrick's main cylinder and the hydraulic hose to the tongs. The rig had fluid leaking onto the ground. One feasible and acceptable method of abatement would be to thoroughly check and repair any leaks on the rig prior to placing it into service according to widely accepted practice in the well servicing industry.

Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: State E., Well No. 20, Crane County: Employees working on a workover unit and engaged in well servicing had not locked out the 440/480 power source to the pumping unit. This condition exposed employees to being struck by the unit should it inadvertently restart without the employees' knowledge or control. One feasible and acceptable method of abatement would be to enforce the lockout standards imposed by the field operator as stipulated and required practice that is widely accepted in the oil and gas industry.

Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: State E., No 20 well, Crane County, Sandhills: An operator of a Franks Explorer III well servicing rig commenced work on the well without installing the H2S monitor on the rig to alert employees to any possible H2S presence in their work area. The operator, representing management allowed the condition to exist. One feasible and acceptable method of abatement would be to re-emphasize to operators and employees that all required components of the rig are to be installed prior to work commencing, especially in known H2S areas.
