Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees required to use Dayton 1 Ton hoists to handle oil field tools and equipment, weighing as much as 500 lbs, were being exposed to hazards associated with falling suspended loads and resultant bodily impacts from these loads, created by use of faulty/deformed hoisting chain hooks as follows: a) In the Tool Shop, the hoisting chain hook was missing the safety latch, the hook opening was spread to 1 5/8" (as manufactured hook opening 1 5/32", hook spread 15/32" or 40.53, which exceeded the manufacturer's and ANSI B30.16-1973 maximum of 15) and the hook tip was bent/twisted with a deflection of 3/8" or 20.556 degrees or 11.42which exceeded the manufacturer's and ANSI B.30.16-1973 maximum of 18 degrees or 10). b) In the Tool Storage area, the hoisting chain hook was missing the safety latch, and hook opening measured 1 1/2" (as manufactured hook opening 1 5/32", hook spread 11/32" or 29.73) which exceeded the manufacturer's and ANSI B30.16-1973 maximum of 15%.mum
