EXECUTIVE ORDER 12196, Section 1-201(j): The head of the agency did not %% operate an occupational safety and health management information system, which shall include the maintenance of such records as the secretary may require: (a) The log of occupational injuries and illnesses (OSHA 2014) or its equivalent, was not completed in the detail provided in the form and the instructions contained therein. Deficiencies found in the 1992 log of occupational injuries and illnesses were: 1) Tuberculosis skin test conversions were not recorded in the log. On or about 10/6/92.
