Section 5 (a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being crushed by adecending vehicle on an
automative lift while
working under the vehicle:
a)  Vehicle maintenance area  The mechanical locking
devices were not in working order on both wayne lifts.
On or about 2/7/94.
Among other methods, one feasible and acceptable method to correct the
hazard is to:
1) Reinstall/repair defective mechanical safety devices (dogs) and
connecting linkage above the floor level.  For assistance refer to
American National Standards Institute (ANSI) B 153111981 Safety
requirements for the construction care, and use of automotive lifts
Section 3.1.10 and Automotive Lift Institute, Inc. Publication "lifting
it Right", Page 16.  section entitled "Lifting the Vehicle".
2)  Instruct and train employees in the proper use of mechanical
safety locking devices for hydraulic lifts.  For assistance refer to
Automotive Lift Institute Inc. Publication "Lifting it Right", page
10 section entitled "Before You Lift".  Also ANSI B1531 Section 6.2.
3)  Conduct Regular/periodic inspection of lifts as recommended by
the lift manufacturer to ensure that the locking devices and all the
other components are in safe operating condition/remove a defective
lift from service until rendered safe to operate.
