employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) On or about August 12, 1994, two ironworkers straddling Beam B-6,
while in the process
of "making" or connecting a double beam connection (beams B-6 & C-6 into
column BB-14)
were not protected from falls and crushing hazards in that Beam B-6 was
not adequately
secured/supported.
Feasible means of abatement include, but are not limited to the following:
a) provide an erection seat or angle in the column web at double beam
connections, or
b) provide a flange length extension on one of the beams and a
corresponding bolt hole in the
web of the column floor or beam.
c) use a come along to secure the beam to the column until the connection
is made.
