Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to toxic gases and vapors while performing work in the Isomax, Alkylation, and Unifiner control rooms. The employer did not provide a ventilation system that met the requirements of National Fire Protection Association Standard for Purged and Pressurized Enclosures, NPFA 496. Among other methods, a feasible and acceptable abatement to correct this problem is: 1)Provide a backup ventilation system with a manifolded compressed air system that will maintain positive pressure during a hazardous gas or vapor release. 2)Provide detectors with alarms and/or interlocks in the control room ventilation system to monitor for entry of flammable and hazardous substances such as hydrogen sulfide, hydrofluoric acid, and flammable hydrocarbon. 3)Provide ventilation ducting away from the building to a location where the presence of flammable/hazardous gases will be minimal. 4)Provide SCBA's in sufficient quantity to allow personnel to work in unknown concentration of flammable or toxic gases. 5)Provide positive pressure indicator to verify the control room's degree of pressurization. 6)Provide the Isomax control room with a readable panel indicator if positive pressure is lost. 7)Provide the HF-Alky operators with portable detectors for hydrofluoric acid.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by falling debris and hit by flying objects due to controls rooms that were not capable of resisting an unconfined vapor cloud explosion or located within the critical radius of such an explosion: a)Isomax control room. b)HF-Alky control room. c)Unifiner control room. Among others, one feasible method of abatement to correct this hazard includes, but is not limited to: 1)Following guidelines for plant layout and spacing. Such guidelines may include: a.The Dow Chemical Company "Fire and Explosion Index Hazard Classification Guide." b.Factory Mutual Loss Prevention Data Sheet, "Spacing of Facilities in Outdoor Chemical Plants." 2)If adequate spacing cannot be achieved, alternative methods of protection must be implemented such as explosion resistant construction or adequate barricading.

Safety relief valves with block valves located as described above include, but not limited to the following: (a) The pilot relief valve located at the Isomax vessel 12v3, Stage I Separator. (b) The pilot relief valve located at the Isomax vessel 12v7, Stage II Separator. Among others, a feasible and useful method to correct these hazards is set forth below: Install "car seals" or use locks and chains to prevent valves from being inadvertently operated as specified in API Recommended Practice 520 or Section VIII, Appendix M, of the ASME Code.

Feasible abatement would be to comply with the National Fire Protection Association, NFPA 85A, Standard for Prevention of Furnace Explosions in Fuel Oil- and Natural Gas-Fired Single Burner Boiler-Furnaces. 1)Provide a convenient observation port on the control side that will allow the fireman to visual inspect the main burner flame or other applicable procedures, such as, utilizing 2 employees during boiler start-up. 2)Provide a reliable control to shut off main flame fuel if ignition has not occurred within 10 seconds. 3)Provide a control that will verify that the supervisor gas cock is closed before starting the purge system. 4)Provide a control that will initiate a safety shutdown upon detection of low gas pressure.
