Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by a falling pumping unit horsehead: Well site; employees engaged in the servicing of an oil/gas well worked on the well's head below the pumping unit horsehead, the horsehead was not secured to the pumping unit walking beam, exposing the employee(s) to the hazard of being struck by the falling horsehead. Among other methods, one feasible and acceptable method to correct this hazard is to ensure that the "throat" bolt be installed in the position provided for it prior to employees being allowed in the area beneath the horsehead.
