were exposed to:
(a)  Plantwide;  Employees were potentially exposed to hazardous
atmospheres
(oxygen enriched/deficient, high levels of combustible gas, etc.) due to
the fact
that the company did not calibrate their environmental monitoring equipment
before each use.
Among others, one feasible and acceptable method of abatement is to have
the environmental
monitoring equipment properly calibrated before each use.

General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
working in Building 86 were exposed to fire and/or explosion hazards.  The
following conditions
contributed to the existence of this hazard:
(a)  There were inadequate methods to detect diborane leaks in the
building, and;
(b)  The standard operating procedure - "Diborane Cylinder Receipt" did not
clearly specify what amount of residue left in a cylinder after weighing
would
require the cylinder to be transferred to the Ice House.
Feasible and accepted abatement methods, among others, include:
1.  Installation of a diborane (Telos) sampling port in Building 86,
(condition a);
and
2.  Updating the "Diborane Cylinder Receipt" standard operating procedure
to
clearly specify at what amount of residue left in a cylinder would require
the
cylinder to be transferred to the Ice House, (condition b).ndition
