Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Production Area, Simon 300 Variable Single Facer; The employer did not implement a preventive maintenance program based on the manufacturer's recommendations on the two overhead hoists used to change the flute rolls. In addition, frequent and periodic inspections were not conducted and records maintained on the two overhead hoists used to change the flute rolls. Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement a preventive maintenance program and conduct inspections with written/signed records.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Production Area, Simon 300 Variable Single Facer; Two overhead hoists were used simultaneously to lift the flute rolls without a qualified responsible person in charge to analyze the operation and instruct all personnel involved in proper positioning, rigging of the load and movements to be made. Among other methods, one feasible and acceptable abatement method to correct this hazard is to assign a qualified responsible person to be in charge of the hoisting operation/supervise complete roll change.
