Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  Outside Shop Area, Room 6, Coffing 1 ton hoist;  The
hoist was not kept in good repair in that:
(1)  The hook was defective (twisted and pitted)
(2)  The controls for the hoist did not function correctly
(3)  The cable to the control pendant was spliced
(4)  The chain and hook were attached to the load in an
unsafe manner
(5)  Employees were permitted on the load
(6)  The hoist was being used as a freight elevator with
only one chain support instead of three
(7)  The supporting structure did not have a load rating
(8)  Load testing was not done with a written report
confirming the load rating
(9)  Frequent and periodic inspections were not conducted
with written, dated and signed reports
(10) Chain and hook inspections were not conducted with
written, dated and signed reports
(11) A preventative maintenance program was not developed
or implemented.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to develop operating and preventative
maintenance procedures in accordance with ANSI B30.16 and
manufacturers specifications.
