Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to the employee in that employee(s) operating a freight elevator were exposed to the hazard of being struck or crushed by the descending car due to the hoistway door interlock being bypassed: a) Basement Bypass plug was used on hoistway door interlock allowing elevator to operate with the hoistway door open. Among other methods one feasible and acceptable abatement method to correct this hazard is to: 1.Remove from the premises any bypass device that could be used to alter the function of the safety interlock on the hoistway door. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to the employee in that employee(s) operating a freight elevator were exposed to the hazard of being decapitation by the ascending/descending elevator car due to the hoistway door vision panel being removed: a) 3rd Floor Hoistway door vision panel was not provided leaving an opening into the hoistway. Among other methods one feasible and acceptable abatement method to correct this hazard is to: 1.Provide a vision panel that meets the requirements of ANSI/ASME 2.6.3(a) through (h) or by protective grills made of 15 gage stainless or galvanized steel Reference Publication ANSI/ASME A17.3-1986 Safety Code for Existing Elevators and Escalators, Section 2.6.3.n
