Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) 2 Lister Avenue Employees were exposed to the hazard of being caught and/or crushed between the elevator shaftway walls or sills. Car gate was not provided on the entrance side of elevator car. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Provide car doors or gates on the entrance side equipped with a car door or gate electric contact. Reference Publication: ANSI/ASME A17.3-1986 Section 3.4.2. BOCA Section 210 S.9.5.
