Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to the employee in that:
Employees were exposed to the hazard of being struck or crushed by
the descending elevator cab due to the electrical contact being
bypassed
on the first floor level hoistway door.
Among other methods one feasible and acceptable abatement method
to correct this hazard is to:
1.  Immediately close the opening in the hoistway door and remove
the screw from the mechanical device used to disengage the electric
contact on the first floor landing side of the hoistway door.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to the employee in that:
Employees were exposed to the hazard of being caught and/or crushed
between the elevator and shaftway walls or sills.  Car gate
interlocks on the north and south side opening on the elevator car
were by-passed using wood blocks, gates were tied back with rope.
Among other methods one feasible and acceptable abatement method
to correct this hazard is to:
1.  Immediately remove the wood blocks from the car gate electric
contacts and release the car gates from being tied back, allowing
the car gate to close and make contact with the electric interlock
device.
