Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: the hazard of having hands, fingers, and/or arms amputated due to inadvertant failure to activate safety and inch machine functions prior to the removal of jammed envelopes from the following machines: On or about 2/24/94, Central Folding Area a)RH-2 Serial #13 b)RH-1 Serial #82 c)RH-3 Serial #87 d)RW-1 Serial #51 e)RW-3 Serial #21 The Knobs/ buttons on all six control panels were painted over with orange paint which made it impossible for an employee to visually verify that any of the functions,including safety features were engaged before placing hand and arms in the machine. ABATEMENT NOTE:: Among other methods one feasible and acceptable abatement method to correct this hazard is to; Remove the paint over legend plates and/or install a legend plate or other comparable means to identify the functions of each control button/knob as stipulated in ANSI B 1691.1-1990 Section 5.2.1(2).
