General-duty citation text

"The following alleged violations have been grouped because they involve
similar or related hazards that may increase the potential for illness".
Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
The employer failed to execute a confined space entry program including
procedures and equipment for appropriate testing, ventilation and rescue,
and training for employees on the procedures and equipment.  On 1/9/93,
the maintenance supervisor entered the vapor phase degreasing tank which
contained atmospheres immediately dangerous to life and health due to the
sludge containing the degreasing solvent 1,1,1, trichloroethane.
Abatement may include, but is not limited to, the development and
implementation of a written confined space entry program, to include at
least the following:
(a)  Require inspection and evaluation by a competent person and limit
of authorization to the competent person including:
1.  Monitoring levels of airborne 1,1,1 trichloroethane and oxygen;
2.  Use of ventilation as necessary to maintain a non-hazardous
atmosphere;
3.  Verify the availability of personal protective equipment,
including respirators and chest or full-body harness; of
rescue equipment; and of emergency services.
(b)  Require a written authorization by the competent person for entry
into the vapor phase degreaser, such authorization to be posted at the
degreaser during entry.
(c)  Establish monitoring and ventilation protocol, including:
1.  Monitoring of the degreaser for oxygen content (must be
greater than 19.5) and 1,1,1 trichloroethane;
2.  The method and extent of ventilation, including continuous
ventilation if appropriate.
(d)  Establish rescue procedures for rescuing employee(s) from the vapor
phase degreaser during cleaning, to include appropriate personal
protective
equipment and rescue equipment.
(e)  Train employees to assure adherance to the above procedures,
including
both the authorized entrance to the degreasing tank and the stand by
attendant.
