Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
On and prior to February 17, 1993, employees on the above site were
subjected to the hazards of either being struck by raised lift/vehicle
or being caught between lift/vehicle and the floor resulting in fractures,
crushing of body parts, multiple internal injuries and/or death due to
inadverent activities of the lift while working under a vehicle supported
by the lift.
Inadvertent activities or movements of the lifts were more likely due to
lack of or deficiencies in safety equipment and procedures as specified
in National Standard Safety Requirements of Construction, Care, and Use
of Automobile lifts, ANSI B-153.1-1981.  Among other methods, one feasible
and acceptable method to correct the hazard is to comply with ANSI B-153.1
along with use of equipment designed in the industry to meet these
standards.  Deficiencies noted were as follows:
(a)  Two in ground, frame contact (free wheeling) Globe lifts with single
lifting post only, and the one drive on, in ground, single post automobile
lift were all being used without mechanical devices or safety latches to
hold them in the fully raised position per 3.2.1 of ANSI B-153.1-1981.
(b)  The one in ground, frame contact Globe lift with auxiliary support
post in addition to single lifting post was being operated with rods of
unknown strength to act as mechanical safety lock.  ANSI B-153.1-1981
Standard 3.2.1 specifies a safety factor not less than 3.
(c)  For all five automobile lifts on site, there were no instructions
on lifting displayed per ANSI B-153.1-1981, Standard 6.2.1.
(d)  For all five automobile lifts on site, there was no regular
maintenance scheduled per ANSI B-153.1-1981, Standard 6.1.
(e)  For all five automobile lifts on site, markings were not available
per ANSI B-153.1-1981, Standard 5.2.  In particular, rated load capacities
of lifts were not available on site.
