Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that on or about 6/12/93,
employees installing a mechanical power press in the Bliss Bay using a
4 point lift system, were exposed to broken bones, internal injuries, or
death due to the shifting or dropping of the load or the lifting beams
during the lifting or the lowering of the load:
Amonth other methods to correct this hazard one feasible and acceptable
method of abatement is to include the following elements:
(1)  Assure that the lifting units on the 4 point lift system are
completely level and plumb at all times during the lifting and the
lowering
of the load by using a level-lift system and/or measuring devices.
(2)  Prior to lifting calculate the pressure required to lift the load
according to the manufacturer's instructions.
(3)  Assure that employees are not in the danger area during all lifting
and lowering of the load.
(4)  Assure that, at a minimum, the operator and the supervisor of the
lift have read and understood the operating instructions prior to
operating
the system and that they are thoroughly familiar with this system and its
limitations.
(5)  Follow the manufacturer's recommendations for servicing and
maintenance of the 4 point lift system.
