Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
On or about 1/22/93, the employer did not furnish employment and a
place of employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to employees
in that employees were exposed to the hazard of serious crushing
injuries from the dropping of a "Rotary" hydraulic automotive lift
while working under vehicles on the lift while the lift was below the
point where the locking devices engage, without supporting the vehicle
with a jack stand at each corner of the lift or equivalent means.  The
lifts had locking devices which could only engage when the lift was
fully extended:
a- One feasible means of abatement is the following:
1- Support vehicles by the use of four jack stands or
equivlanent means for each vehicle under which an employee
is required to work.
2- Instruct and train the employees in the proper use of
the manufacturer's mechanical locking devices, were provided,
and the safe operating instructions of the lifts.
3- Conduct requured maintenance procedures described in by
the manufacturer.
4- Conduct the recommended inspections of the lifts by the
manufacturer to ensure the locking (safety) devices and all
other operaing components are in safe operating condition.
Remove defective lifts from service until rendered safe for
operation.
5- Post and maintain permanently on display the
manufacturer's safe lifting instructions in the work area.
Locations of unsafe "Rotary" lifts devices observed on 1/21/93.
All of the lifts in this shop have locking devices
that operate only when the lift is at full extension.
