Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On or about 2/2/93, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of entering spaces: a- An employee enters confined spaces without all appropriate means being provided for safe entry, such as, but not necessarily limited to testing of the breathing atmosphere and provision of rescue equipment. The employee goes into either of two 60,000 gal. open-top waste treatment anks for maintenance and repair. Condition noted on or about 2/2/93.
