Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to a fall hazard and serious injuries while installing metal
sash within a window wall opening while standing on the sill and step
ladder above the floor level without any fall protection:
(a)  Jobsite, 2nd floor, Front:  Employee was exposed to fall
hazards and serious injuries while installing metal sash within
a window opening standing on the window sill and step ladder
without any fall protection.
Among other abatement methods would be used, a safety belt and
lanyard, install a guardrail across the width of the opening
or stand or a mobile scaffold platform with guardrails to prevent
employee from falling out the the opening to the exterior.
