Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Hardware Department, Building number One: Employees were exposed to a fall hazard at the top level of the inclined lift when the carrier was at the bottom level. Employees, while moving boxes around, came within one to two feet of the open edge of the floor, exposing them to a nine foot, eleven inch fall to the concrete floor below. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to install interlocked doors so that they can be opened only when the carrier has stopped at that level and the carrier cannot be moved until they are closed; in accordance with ANSI Standard B20.1-1990, paragraph 6.6.2(b).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Hardware Department, Building number three (3): Employees were exposed to crushing injuries at the bottom of the inclined lift area when the carrier was coming down. Employees, while working at the bottom level, could get parts of their bodies in the path of the carrier as it descended. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to install a suitable enclosure on all sides of the path of the carrier and equip the enclosure with an interlocked door to prevent employees from getting any part of their body in the path of the carrier until it is stopped at the bottom and only then doors could be opened; in accordance with ANSI Standard B20.1-1990, paragraph 6.6.2(b)(c).
