Occupational Safety and Health Programs for Federal Employees:
the employer did not furnish to employees places and conditions of
employment that were free from recognized hazards that are causing
or likely to cause death or serious physical harm in that employees
were exposed to potential unexpected exposure to toxic,
flammable/explosive, or oxygen enriched or deficient atmospheres
and/or biological or mechanical hazards while performing servicing,
maintenance, inspection or similar activities in confined spaces:
a)  Facility - Employees, such as mechanics and machine
operators, were permitted to enter confined spaces, such as
sandblasting pits or tunnels, without benefit of pre-planning
for entry, worker instruction programs, and rescue procedures,
as applicable, being instituted.
One feasible and acceptable method, among others, to correct
this hazard is to institute a formal Confined Space Entry
Program such as, but not limited to programs outlined in
ANSI Z117.1 - 1989; ASTM D4276 - 84; proposed standard
20 CFR 1910.146, Federal Register dated 6/5/89, and
NIOSH Publication 80-106.  Or, implement and enforce
the recommendations of the local activity's procedure "entering
Tanks and other Enclosed Spaces."  On or about 1/24/92.
