Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:

a) The hazard of being struck by exploded billets
which can be ejected from hydraulic extrusion presses
during implementation of failed billet procedures
which utilize blocks of wood between the ram of the
press and the billet stuck inside the container which
can drive the wood into the billet under ram pressure
and cause build up of gases inside the stuck billet
creating an explosion hazard.

One feasible and acceptable abatement method to correct
this hazard is to implement the failed billet procedure
recommended by the manufacturer of the Sutton presses and
contained in their instructional manuals which came with
the presses

Also discontinuances of wood or combustible materials that
can penetrate the billet under ram pressure and create
an explosion potential.

All affected employees in the Extrusion Dept on all
presses should be thoroughly trained in the failed
billet procedures that the manufacturer recommends for
extrusion presses.  On or about 3/5/92.
