The alleged violations below have been  grouped  because  they  involve
similar or related hazards that may increase the potential  for  injury
resulting from an accident.
Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  On or about October 16, 1992, employees required to enter
manholes and storm drains were exposed to oxygen deficiency and
hazardous gases during cleaning operations. A feasible and useful
method to correct this hazard is:
1. Assure that the atmosphere inside of the confined space
tested for oxygen deficiency and the presence of gases prior to entry.
(b)  Among other methods, one feasible and acceptable abatement
method would be to follow the procedures for confined space entry
described in ANSI Z 117.1-1977 and NIOSH Publication Criteria for
Recommended Standard - Walking in Confined Spaces.
