employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:

helpers were exposed to welding fumes including vanadium pentoxide,
several employees reported sick. An immediate effort was not made
to determine the cause of the illnesses, provide appropriate
personal protection or remove employees from the work site.
