Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to the hazard of being struck by structural members and process
equipment:
(a)  Saturation area - The reinforced concrete slab ceiling on the
second floor above the break room and also above the C-405 brine
pump, the L-504 spare acid brine pump and the C-419 spent brine
pump were disintegrated in several areas and reinforcement bars
were visible, on or about 3/29/92.
(b)  Salt Plant - The Number 1 brine clarifier tank used for filter
waste from the Salt Plant was corroded and leaking around the
circumference of the metal tank along a seam towards the bottom,
rested on horizontal I-beams upon four (4) concrete piers, had
several I-beams that were corroded to the point of having lost
substantial parts and had some cracked concrete piling(s), on or
about 3/29/92.
(c)  Salt Plant  -  Structural beams, columns  and  connections in
proximity to failed salt bulk loader tank BL Number 1 were severe-
ly damaged  and  the support for  BL Number 2 was greatly reduced
by corrosion, on or about 3/29/92.
(d)  Number 1 Cell Room - The upper section of block wall was separated
from the lower section of block at the salaried break room at the
west end of the room on the second floor, on or about 3/29/92.
(e)  Bleach Plant - At the injection area platform above the reaction
tanks, the hanger for a 1-inch caustic injection line for B-100
caustic injector weak gas was disconnected and there were no
hangers or supports for the 1-inch lines for the B-101 caustic
injector weak and strong gas, on or about 3/12/92.
(f)  Salt Plant  -  A 1-inch air line west of the brine filter aid mix
tank had a hanger disconnected  and  the 4-inch chlorinated brine
air blow tower piping  did not have hangers or supports, on or
about 3/12/92.
(g)  Products Area - A 1-1/2 inch fiberglass muriatic acid (dump) line
from the  Number 4 HCL burner  had two (2) hangers disconnected, a
1/2-inch hydrochloric acid line (for circulation on the scrubber)
did not have hangers or support, a 1/2-inch hydrochloric acid line
to Number 4 burner tails tower was not provided with support, and
a 1/2-inch air line at the sulfuric acid storage tank dike did not
have hangers, on or about 3/18/92.
(h)  Boiler House - A 1/2-inch water line for the emergency hydrogen
shut-off water flood did not have hangers or supports, the mercury
bleed-off trap piping by the separator at the feed line to the
hydrogen boiler was not provided with supports, a 1/2-inch air
line at the south side of the hygdrogen boiler had supports
missing and the constant flow blow-down steam lines at the north
side of the hydrogen boiler were missing hangers or supports from
the damper to the west column, on or about 3/19/92.
Among other methods, one feasible and acceptable method to correct this
hazard is:
(a)  Upgrade the structures back to their original condition and
maintain pipeline systems with enough support to maintain the
piping.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to the hazards of contacting hazardous chemicals:
(a)  Plant - Pipelines and valve outlets for piping systems were not
all labelled, including but not limited to chlorine, hydrogen,
mercury, brine (chlorinated, saturated), acids (hydrochloric,
sulfuric), caustics (sodium hydroxide, sodium hypochlorite),
propane, diesel fuel, water (raw, city, deionized), and steam,
on or about 3/11/92.
(b)  Number 1 Cell Room - The 6-inch cell brine lines had 4 of 8 bolts
missing from their flange couplings, there were 3 of 8 flange
bolts missing for cell 13 strong gas chlorine valve at the one-
half inch suction, 2 of 8 bolts were missing for cell 14 weak gas
valve flange at 4-5 inch suction, 5 of 8 bolts were missing for
cell 15 strong gas valve flange, 4 of 8 bolts were missing at the
trap-end spent brine drop-tube flange coupling for the 6-inch
gravity flow drain, and at the lower level, a flange coupling for
a 10-inch gravity flow spent brine line had 12 of 24 bolts mis-
sing, on or about 3/11/92.
(c)  Products Area - At the walkway to the chlorine receivers, the
chlorine venting manifold T-flange couplings by Number 2 chlorine
receiver were mismatched and jerrybuilt with a 4-bolt flange
bolted to an 8-belt flange and two 8-bolt T-flanges of different
bolt patterns bolted together with 4-bolts in a redesigned pattern
(re-machined), 11 of 16 bolts were missing from the gate valve
flange coupling for the Number 1 Tarrier chlorine unit vent
chimney for the rupture disk as well as 12 of 16 bolts missing for
Number 2 and 8 of 16 missing for Number 3, on or about 3/18/92.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
(a)  Identify piping system contents in accordance with ANSI "Scheme
for the Identification of Piping Systems",ANSI A13.1, and use the
number of bolts for flange couplings in accordance with the bolt
pattern.

(a)  Salt Plant - The CATERPILLAR front-end loader (Number 1) was
operated with an inoperable backup alarm, a cracked rear window
and a frozen emergency brake to move salt and to load a hopper
for a bucket conveyor system, on or about 3/11/92.
(b)  Salt Plant - No back-up alarm or signalman for WHITE-GMC dump
trucks Numbers 4041/601712 and Number 4051/601711 used to deliver
salt and both vehicles had hydraulic leaks for the dump
mechanisms, on or about 3/12/92.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
Always  inspect  the  equipment  prior to use and correct any
deficiencies prior to operation.

(b)  Salt Plant - The CATERPILLAR front-end loader (Number 1) was used
to move salt and load a hopper for a bucket conveyor system and
the operator did not use the seatbelt, on or about 3/11/92.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
(a)  Require the use of seatbelts.

(a)  To provide pipeline system station outlets with different union
connections such that a hose for air or water will not connect
to a steam pipeline system station outlet.
