Section 17(a) and Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of toxic chemical inhalation from an uncontrolled release of titanium tetrachloride: (a) Chloride Plant, Oxidation - Employees were exposed to the hazards of toxic chemical inhalation, chemical burns and zero visibility due to the uncontrolled release of titanium tetrachloride. The response to system control, visual and audible computer screen alarms, showing an overfill of the C-solution tank with titanium tetrachloride on 6/3/92, did not initiate a prompt pump shutdown to prevent overfilling and subsequent ruputre disk failure that allowed venting of approximately 1100 lbs. of titanium tetra- chloride into the atmosphere. Kemira Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent Standard Section 5(a)(1) which was contained in OSHA Inspection Number 17402785, Citation Number 01, Item Number 01, which was issued on 6/11/90.
