Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of being struck or crushed from
failure of hoisting
equipment:
a)  Rear (west side) outside Fab Building and all other hoists on the
property
employees use an electric hoist that is mounted to the underside of a
steel beam to lift
large objects.  Hoist does not have a rated capacity marked on it; no
warnings to the
operator displayed; when idle for more than one month, frequent and
periodic
inspections are not performed before being placed in use; inspection and
maintenance
records are not available; limit switches, before use, are not tested; and
operators are
not given a practical operating exam on this unit.  Any employee is
permitted
to use
the hoist when needed, on or about 5/9/94.
Among other methods, one feasible and acceptable method to correct this
hazard is to
implement and follow manufacturer's procedures or the American National
Standard
Institute (ANSI) B30.16 for Overhead Hoists requirements for inspections,
labeling,
operator training, and maintenance.

employees were exposed to the hazard of amputation when operating the
Piranha (Iron
Worker):
a)  Fab Shop, building #3 - employees using and maintaining Piranha
(punch/shear)
Iron Worker were not trained to ensure the original and continuing
competence of
personnel caring for, inspecting and maintaining Iron Workers; records for
periodic
and regular inspections and maintenance work performed were not
maintained; the
main power disconnect switch was not locked off for any inspection or
repair, only
the machine switch was turned off; the point of operation was not made
inoperative
before adjusting and aligning dies; there was not a posting of the
procedure spelling
out the actuating of the ram shall be prevented for the purpose of making
the point of
operation inoperative; and operators were not trained in the safe method
of work
before starting work on any operation as evidenced by suing the unit
without point of
operation guarding for the punch and shear portions, on or about 5/9/94.
Among other methods, one feasible and acceptable method to correct this
hazard is to
implement and follow manufacturer's procedures or the American National
Standards
Institute (ANSI) B11.5 for the care and use of Iron workers for
maintenance,
inspections, operator training, and operational procedures.
