Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to in that employees were exposed to crushing and amputa- tion from unintended operation: (a) Press Department - Press brake M-11 had no treadle guard to prevent accidental activation of the machine, on or about January 15, 1992. (b) Press Department - Verson press brake, serial nubmer 24012-308-90, 90 tons mechanical, with no treadle guard to prevent accidental activation of the machine, on or about January 15, 1992. (c) Frame and Rail Department - Thirty (30) tomkins and Johnson riveting machines had not guard on the foot pedal to prevent accidental activation of the machine, on or about January 15, 1992.
