hazards of operating machinery when a mechanical condition of the
equipment is discovered which would
be likely to produce a hazard to the operators by reason of carbon
monoxide poisoning:

Driver employee removing snow from the facility operated a Hough Model H90
Payloader Serial Number 17954 where the exhaust system was missing from
the engine
compartment.
AMONG OTHER METHODS, SOME FEASIBLE AND ACCEPTABLE ABATEMENT METHODS
TO CORRECT THIS HAZARD ARE:
1.INSTALLATION OF A MANUFACTURED SPECIFIED EXHAUST SYSTEM.
2.REMOVE FROM SERVICE ALL EQUIPMENT WHICH HAS MECHANICAL
DEFICIENCIES WHICH WOULD BE LIKELY TO PRODUCE A CARBON MONOXIDE
HAZARD.
3.IMPLEMENT ONGOING MAINTENANCE AND REPAIR INSPECTION PROCEDURES
FOR ALL EQUIPMENT WHICH HAS CARBON MONOXIDE GENERATING
